Our Services
Consumer Directed Service Facilitation and Community Health Support
We assist individuals who have received approval to participate in Medicaid’s consumer-directed model of non-medical home health care. Upon approval, we guide individuals through the selection and recruitment of their caregivers who will provide personal and/or respite care. Additionally, we provide case management and other community-based health-oriented services.
Advocacy
Assistance is provided with navigating the Medicaid long-term care intake application process and other health matters that may lead to significant health challenges if left unaddressed.
Limited Financial Assistance
On a case by case basis, we offer financial assistance to individuals and/or families in dire need of food assistance or other life-sustaining services. However, due to limited resources, applicants must live below the federal poverty guidelines. In special circumstances, consideration will be given to those with income that exceeds the guidelines.
